Brief Job Description (essential functions of the job):

 

Reporting to the Service Delivery Manager or designee, identifies and manages relationships with an array of human and social service providers to facilitate access to services and opportunities for high-risk and proven risk young people who are engaged with SOAR (Street Outreach, Advocacy and Response) Boston.

Responsibilities:

  • Works with proven-risk youth and young adults to successfully reach employment, education and life goals.
  • Assists participants with obtaining resources for basic needs such as food, clothing, and health care according to departmental policies and procedures.
  • Maintains contact with service providers and employers to ensure retention of participant and adequate support; documents cases according to departmental policies and procedures. Confers complex cases to Service Delivery Manager as required.
  • Works with the Service Delivery Manager to facilitate basic job-readiness and life-skills trainings for participants, as needed.
  • Cultivates relationships with employers to assist participants in attaining employment. In partnership with an Outreach & Engagement Coordinator; develops a service plan for participants. 
  • Provides support to participants in meeting their goals along a path of behavioral change.
  • Maintains records and documentation electronically; collects and enters information and completes forms and/or reports according to departmental policies and procedures on department-issued devices.
  • Attends scheduled meetings, trainings and workshops.
  • Performs related work as required.

Minimum Entrance Qualifications:

  • At least three (3) years of full-time, or equivalent part-time, previous work experience in the youth services field in work which included case management and/or program development.
  • A bachelor’s degree in criminal justice, social or human services or related field may be substituted for two years of the required experience.
  • Demonstrated ability to work with or on behalf of high-risk urban youth and young adults.
  • Knowledge of array of services available to high-risk youth populations in Greater Boston highly preferred.
  • An understanding of the diverse and complex issues facing gang involved and proven-risk youth.
  • Ability to de-escalate situations and maintain professional demeanor in stressful situations required.
  • Ability to work independently while also maintaining strong collaborative relationships with other team members and external partners.
  • Proficiency with computer software such as databases and Microsoft Office.
  • Ability to build trust and maintain confidentiality.
  • Ability to exercise good judgment and focus on detail as required by the job.

 

Must have and maintain a current Massachusetts Driver’s license.
